Your Medical History

We require an up-to-date Medical History to avoid medical emergencies and prevent dangerous drug interactions during your dental procedure.

In addition, there are regulatory requirements that our practitioners must abide by for the delivery of dental care in Australia. This means you will still need to update your Medical History, even if you have had no changes in your health since the last time you visited.

The Dental Board of Australia is a national statutory body that registers and regulates dental practitioners and students across the country under the National Registration and Accreditation Scheme. Its primary role is to protect the public by setting professional standards, codes, and guidelines.

The Dental Board has a mandatory Code of conduct that all dental practitioners must follow. This crucial document sets the standards and regulatory guidance for dental practitioners, including the upkeep of dental records (or ‘health records’ as described in the code). The Code of conduct expects dental practitioners to check and record medical status changes before starting any new treatment.

Our practitioners are likely to ask you verbally if there have been any changes to your health when you visit every 6 months. In addition, you can update your Medical History annually by either:
- Following the secure online portal link when you have made an appointment OR
- Filling in an updated Medical History form when you check in before your appointment.

If you are unable to update your Medical History on an annual basis, this will be documented in your treatment records, and your dental provider will make the individual decision on what treatment they are able to provide that will not compromise your health.
We thank you for your understanding and co-operation in the delivery of your dental needs.
Yours Sincerely,
Drs Cathy Findlay and Michael Cooke
